Students New to the Madison Metropolitan School District
For any student new to the Madison Metropolitan School District (MMSD), online enrollment must be completed at www.mmsd.org/enroll and the online enrollment print-out should be taken to the home attendance area school to complete enrollment. Parents/guardians may look up their home attendance area school at https://webapp1.madison.k12.wi.us/webapp1/find_your_school/
4K and 5K Registration will be held in March 6th, 2017 from 2pm to 6pm at all MMSD Elementary Schools. If you are unable to attend that day, you may still start the enrollment process by visiting www.mmsd.org/enroll. Print these forms and take them to the home attendance school to complete enrollment.
*Hmong Developmental Bilingual Education is not a program option for 4 year-old kindergarten. For information on 4K, please visit: www.mmsd.org/4K
Students Currently in the Madison Metropolitan School District
Although students currently in MMSD’s 4K programs are already enrolled in the district for kindergarten, you must complete the interest form below for consideration into the Hmong DBE program. This interest form will also serve as an internal transfer application if you reside in the MMSD area, but currently reside outside of the Lake Vew Elementary School attendance area.

Interest Form Process
The Hmong DBE Program is open to all MMSD Hmong English Language Learners; however, it is only available at Lake View Elementary School for incoming Kindergartners for the 2017-2018 school year.
Incoming Kindergartners identified as Hmong English Language Learners in the following school attendance areas may submit an interest form for the program and will be provided transportation from the district:
Lake View Elementary School
Lindbergh Elementary School
Gompers Elementary School
Mendota Elementary School
Hawthorne Elementary School
Hmong English Language Learners entering Kindergarten who live outside of the school attendance areas above can also submit an interest form for the program, but will need to provide their own transportation.
Attendance Area Preference
Preference is given to students in the following attendance areas:
Preference is given to students residing in the feeder school attendance areas mentioned above (Lake View, Lindbergh, Gompers, Mendota, and Hawthorne).
Preference is given to students residing in the MMSD attendance area.
Parents/guardians must complete an interest form which must be submitted to be considered for participation in the program. Completing an interest form does not guarantee that your child will be admitted to the program. Space is limited.
It is very important that parents/guardians understand the goals of the DBE program and that by enrolling your child, you understand the benefits of having your child be in the bilingual program for as long as possible. Parents/guardians who have not discussed the Hmong DBE program with staff from the Office of Multilingual and Global Education will be required to attend an informational session.
Interest Form Deadline
For the 2017-2018 school year, the Hmong DBE Program interest form submission deadline is Friday, April 28, 2017.
Parents will be notified regarding their status of admission to the Hmong DBE Program on or before May 10th, 2017, by phone. Please be sure to include a working/preferred phone number on the interest form.
